I, the undersigned employee or prospective employee of Archer Mechanical, Inc. hereby acknowledge that I have received a copy of the drug and alcohol policy of the Company. I also agree to comply with the substance abuse policy as a condition of employment with the Company. Further, I also understand that this agreement does not create an obligation or contract of employment between Archer Mechanical, Inc. and myself.

Further, I consent to any request under the Policy for a specimen for the purpose of detecting the presence of drugs or alcohol and authorizing its designated service provider to collect the specimen.
